The DD Form 214 is a complete and thorough document that verifies a service member's proof of military service. This Certificate of Release or Discharge form is issued through the Department of Defense and is typically issued at the final outprocessing appointment prior to retirement or separation.
U.S. Department of Veteran's Affairs has all the different methods to obtain your own or a family members military records to include DD-214 or other military service records.
For active duty members as its illegal to photocopy military identification cards, so we accept any travel orders that include your name in place of a DD-214.
For security purposes, please black out the Social Security Number on your official documents before submitting.
I'll help my fellow veterans by becoming a member of The American Legion. I certify by forwarding this application that I served at least one day of active military duty during the dates marked below and was honorably discharged or am still serving honorably.
